Calvary Kids Registration Form

I hereby give full consent for my child, a minor, to attend the Calvary Christian Center Calvary Kids ministry program. I hereby assume all risks involved with my son/daughter participating in the program. I hold the church and its staff members/volunteers blameless from any liability, actions, cause of action, debt, and claims provided by the church. I also consent for my child to be a part of any media collection (photography and video) pertaining to Calvary Kids ministry program and release all rights to the church media collection. I understand and give consent to Calvary Christian Center to use the following information for my/our pastoral care, emergency care, and participation in church activities. I understand that my information will only be given to pastoral and church staff, Calvary Kids volunteers, and emergency personnel on a need to know basis. By checking off this Consent form, I indicate that I have read, understood, and approve the above and that this information is accurate.
